Instructions for scoring
Add the scores for each item 1-5 (total possible score = 25).
ED severity classification
Total score 5-10 (severe); 11-15 (moderate); 16-20 (mild); 21-25 (normal). An aggregate score of the 5-items scale can be calculated and used to classify the severity of the disorder.
The questions above should only be completed by individuals who have been sexually active and have attempted sexual intercourse in the past three months. For sexually inactive individuals, the questionnaire may be answered for the last period of time (three months or longer) during which the individual was sexually active.
